On 23 September 2018, the Government of India introduced an ambitious government-sponsored health scheme. This is the term the Pradhan Mantri Ayushman Bharat Yojana (now called the Pradhan Mantri Jan Arogya Yojana (PMJAY). The plan was introduced with the intention to reduce the financial burden on the poor.  And vulnerable groups as a result of hospitalization and ensure they get access to quality healthcare. 1. Age proof document: It is required as proof to ensure whether the person is responsible enough. This is to enroll his or her family in this scheme or not. 2.Identification Details: To be enrolled in this scheme, candidates are required to produce their identification details as well. They can use their Aadhar card and PAN card as a source for identification.

4.Family Structure: the Family structure is to be considered while enrolling for this scheme. By family structure, it means whether the family is joint or nuclear.
